All students entering 7th grade are required to submit documentation that they are up-to-date on their immunizations. Students must have the TDAP Booster and submit the proper documentation with the first week of school in order to remain on campus. Proof of vaccination must be submitted on the TN Vaccination form (see example below). Forms can be submitted on paper, by fax at 615-333-5633 or emailing directly to [email protected].
